Certificate transfers
Please note that the following information will be required for certificate transfers not requiring additional auditing:
- A copy of the most recent management system certificate
- A copy of the initial assessment report or the last recertification assessment report, whichever is most recent;
- A copy of all surveillance assessment reports since the last recertification assessment or initial assessment, whichever is most recent;
- A copy of all finding associated with the above assessments;
- A list of complaints received and actions taken in the last 12 months;
- A copy of the most recent audit plan from the current certification body showing the plan for the full cycle;
- Information on any current engagement of the client with regulatory bodies that is relevant to the scope of certification in respect of legal compliance.
- A copy of current quality manual or similar, defining quality system and policies
- Reason for seeking a transfer of certification

Declaration and Submission
In submitting this quote request to Eurofins E&E CML Limited and CML B.V. The applicant attests that:

· The rights to any certificate issued shall rest with the certificate holder stated.

· They have the authority to represent, submit the request, agree terms on behalf of the applicant.

· The signatory is aware of and is able to make a declaration on behalf of the company. The undersigned agrees that the company will comply with the Certification Agreement (rules and regulations) of the applicable Eurofins E&E CML Limited or CML B.V. certification scheme.

· Unless otherwise stated in this quote request, that no consultancy service has been used in the design or maintenance of the management system applicable to this application.
