Swim Lessons Contract

All swimmers should be dressed in a swimsuit and ready to begin at the scheduled start time. Lessons will end at the scheduled time regardless of start time. The swim instructor who is late for a lesson should add on the missed amount of time to the end of the lesson.

Any client who does not show for a lesson will be charged for the appointment.

As a professional courtesy there will be no charge on lessons cancelled with a 24 hour notice. Any cancelation less than 24 hours will be considered a “no show” and the client will be charged for the lesson. Prepaid lessons will remain on your account to be used for a later lesson. We will take into consideration illness or last minute emergencies.

I understand, agree, and acknowledge that there are risks inherent in sports activities, including swimming, and including any swim class taught by Danila Novikov. These risks include, but are not limited to, paralyzing injuries, brain injuries and death. These activities may be of a hazardous nature and/or may include activities such as swimming, diving, a variety of strenuous exercise and physical activities. With the full understanding of the facts, I state that to the best of my knowledge I, or my son/daughter/relative listed above, has no medical, physical, or emotional health conditions, which would hinder or prevent his/her participation in a swimming class. I also understand that in- water lessons cannot commence unless this form is signed by the adult student or an adult guardian of a student.

At Danswim, all lesson purchases are non-refundable after 14 days from the date of purchase. We enforce this policy to maintain the integrity of our scheduling and to ensure that our coaches and resources are appropriately allocated. However, under certain exceptional circumstances, we may consider issuing a refund after the 14-day period. These situations will be evaluated on a case-by-case basis, and any refund granted will be at the sole discretion of Danswim management based on what we believe is appropriate and fair.

Clients consent to the use of photographs/videos taken during activities for [Your Facility's Name]'s promotional purposes, subject to privacy considerations and an opt-out option.
