BOOKING & CONSENT FORM

I understand that this procedure requires single synthetic fibers to be adhered to my own natural lashes and that it is my responsibility to keep my eyes closed and be still during the entire procedure until my lash technician, Janie Toups, addresses me to open my eyes. 

I understand that some risks of this procedure may be but not limited to eye redness and irritation. The fumes from the adhesive may cause my eyes to tear up if I open my eyes.

 I agree to disclose any allergies that I may have to latex, surgical tape‘s, cyanoacrylate, Vaseline etc. as well as any medical conditions or medications that may have symptoms or side affects such as hyperactivity, inability to sit still for long periods of time. 

 I will seek medical care, at my own expense, and contact Janie Toups, immediately if any allergic or adverse reactions occur.

I understand that by not following the lash extension aftercare provided that I may be at risk of build up and/or bacteria along the lash line. This may prevent my lash technician from applying lash extensions properly. I agree to the fees that may be applied for removal and cancellation.

I give Janie Toups permission to show my before and after photos of my lashes to other potential clients and to social media platforms.

 I understand that lash extensions require ongoing maintenance and fill fees are based on time and or the number of extensions that need to be replaced at the fill appointments. If I wait too long before fills I made need to pay for a new full set. If I no longer wish to wear lash extensions, my technician will remove them, I will not try to remove them myself to avoid damage.

I release Janie Toups from any and all liability associated with this procedure which will be performed with the upmost attention to safety and proper application using tools and products that the technician has been trained to use. This procedure has many variables such as lifestyle, moisture, weather, extreme temperatures, natural lash shedding and other factors.

The technician will assess and decide if I am a candidate for the service to the best of their ability. No guarantees are made or implied. The outcome of each set varies on the state of the clients natural lashes.

I understand that the appointment is located at a private residence and no guests or children are permitted to come to the appointment. The service will be rescheduled if guests are brought and a cancellation fee will be applied.

CANCELLATIONS & REFUNDS

50% of service will be charged for all cancellations, within 48 hours of booked service. 

Clients who are 15 minutes late will be charged a $10 fee.

All no shows are charged 100% of the service.

Clients who cancel their appointments 2 times consecutively will be required to pay for future services at the time of booking. No refunds.

$10 fee is charged for rescheduling within 48 hours.

Clients who do not pay the cancellation/rescheduleing/no show fee will be banned from booking any appointments until balance has been paid.

Janie Toups does not give refunds under any circumstances. The card on file will be charged.
