I, the undersigned, authorize the United Brotherhood of Carpenters and Joiners of America (UBC) and its affiliated councils/locals and the Carpenters International Training Fund and its affiliated training funds to display and share for business purposes the confirmation I have provided of my completion of the Covid-19 vaccine and any subsequent booster shots that may arise.

I understand this confirmation will be visible and accessible in the TRAIN and Personify systems and through my Training Verification Card (TVC).

This information may be used, for example, to qualify me for work opportunities if access to the job requires it, or to verify eligibility to attend training programs, either locally or at the International Training Center.

Please upload a copy of your Covid Vaccination Card below. Make sure document is clear and none of the information is blocked. Once documentation has been verified, your TVC card will be updated.
