This form is for US participants who will be registering online and mailing in their entry.

Please ensure participant's full name is on the back of the artwork.
Eligibility:
- The Fish Art Contest is free and open to all persons in grades K-12 (aged 5-18 years old.)
- Only one entry is allowed per person, previous entries are not eligible.
- An entry consists of an entry form, artwork and creative writing submission (writing required for grades 4th-12th.)
- Entries must abide by the guidelines and postmarked to

I have read the complete “Rules and Guidelines” for this year. My artwork is HORIZONTAL, 9”x12” and is no more than ¼” thick. My writing is no more than one side of one page in length and has myname, grade & state on it. (Writing required for grades 4-12) My entry form is complete, easy to read and signed by myself and an adult.
