Use this form to request to borrow the Branch trauma/bleeding kit for your Club's ART training course. Your booking is not confirmed until the Branch office has replied by email.
Trainers & CTOs please note:

HLTAID011 Provide First Aid (PFA) is now a pre-requisite for the Advanced Resuscitation Techniques course which has been updated to align with HTLAID015.

All course participants should hold the PFA before attending the ART course.

If this is not possible, they MUST complete the PFA before attending an ART assessment and it must be on their record. ART candidates who do not meet this requirement will be turned away from the assessment.

Clubs should provide the Branch office with a Form 14 (generated out of Surfguard) for the ART course when confirming candidates into an assessment.
