Tattoo Appointment Request

Please fill out this form to request a tattoo appointment. Once I have reviewed your form and determine if I will be proceeding with your tattoo, I will respond with deposit instructions and an available appointment date.

I respond to emails at least once a week, so there is no need to message me to tell me you've filled out a form unless it has been over a week. Please check your junk folder!

Detailed description of tattoo including style, colour/greyscale/linework, size (in inches or cm), placement, and budget if any. The more information I have to go off of the better I will be equipped to create your piece the way you want it.

Your availability (any days that work better for you, or don't work for you.) I work Mon - Thurs starting at 12:30pm.

Please submit any relevant reference photos for your tattoo below. If you want one of my flash pieces, drop it here as well. I don’t copy other artist’s work.
