KEANNASLASH: CONSENT FORM

I agree to have eyelash extensions applied to my natural eyelashes and/or removed and retouched. By signing this agreement, I consent to the placement and/or removal of the eyelash by the certified eyelash extension technician. I further understand and consent to having my eyes closed and covered for the duration of the approximate 3-3.5 hours procedure.

I understand that in rare occasions, there are risks associated with having artificial lashes and lash extensions applied to or removed from my natural eyelashes. I understand the irritation to the orbital eye area, including the eye itself, stinging, discomfort, blurring if the adhesive enters the eye, and the possibility of an allergic reaction can occur. I agree that if I experience any of these conditions during the procedure or after that I will let the certified eyelash extension technician known immediately and it may be beneficial to have the eyelashes removed.

I understand my eyelash extension technician will take every precaution and will use lashing techniques to minimize or eliminate negative reactions as much as possible. I also understand that if my eyelash extension case requires extra concern, extra time will be needed to complete my procedure.

I understand and agree to the after-care instructions provided by the certified eyelash extension technician for the use and care of my eyelash extensions. I realize and accept the consequences of failure to adhere to these instructions may cause the eyelash extensions to fall out and/or decrease the time the extensions will last.

I understand and agree that there are absolutely no refunds to any services offered by KeannasLash including any allergic reactions. Furthermore, I understand that if there should be any concerns regarding to the service performed, I must contact KeannasLash within the next 48 hours to resolve the issue. I understand that if I am outside the 48 hours window, it is up to me to book any services that are needed to fix the issue.

This agreement will remain in effect for this procedure and all future follow-ups conducted by the certified eyelash extension technician. I understand that this consent agreement is legal and binding. I understand the procedure and accept the risks. I consent to the agreement and to the eyelash extension application procedure.
