COVID-19 Liability Release Waiver

Due to the 2019-2021 outbreak of the novel Coronavirus (COVID-19), The Prophet's Daughter is taking extra precautions with the care of every client to include health history review and enhanced sanitation/disinfection procedures in accordance to Mississippi State Law.

Symptoms of COVID-19 include:
Fever
Fatigue
Dry Cough
Difficulty Breathing
Loss of Taste and or Smell
Diarrhea
Runny Nose

I agree to the following:*

I have not experienced symptoms that of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or any communicable disease within the last 14 days.I have not, nor any member(s) of my household, traveled by sea or by air, internationally within the past 30 days.I did not, nor any member of my household, visit any any area within the United States that was reported to be highly affected by COVID-19, in the last 30 days .I have not been, nor any member(s) of my household, diagnosed to be infected of COVID-19 virus within the last 30 days.I understand that The Prophet's Daughter LLC cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.I understand that I am required to wear a mask upon entering the salon, and required to wear the mask during the time I am at the Prophet's Daughter Salon. If I refuse to wear a mask, I will be asked to leave and there will be a full charge for the cost of my appointment. If I am unable to wear a mask due to a health condition, I will be required to reschedule an appointment. There are masks available for purchase if I need, or I may wear my own.I understand that there is a 24 hour cancellation notice required on my reservation. There is a 100% charge if I do not reschedule or cancel my reservation within 24 hours. Please note that the deposit is nonrefundable UNDER ANY CIRCUMSTANCES. There must be another deposit made to reschedule.

By signing below I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation. This waiver will remain effective until laws and mandates relevant to COVID-19 are lifted.
