Cancellation Policy
If you are unable to keep your appointment please contact us within 12 hours prior to the scheduled time to cancel or reschedule your appointment. The first "no-show" or last minute cancellation is subject to a $50.00 fee, the second occurrence and beyond will be billed for the full amount of the scheduled appointment.
Payment Policy
All payments are due at the time of service; Fusion Therapeutic Bodywork does not provide any billing services. Cash, credit cards, Venmo, and Paypal are gladly accepted.
Consent for Treatment
If I experience any pain or discomfort during the session, I will immediately inform the practitioner so that the pressure or technique may be adjusted to my level of comfort. I understand that it is recommended that I see a doctor, chiropractor, or other medical specialist for any medical or physical ailment that may need attention. I further understand that massage therapy and bodywork should not be construed as a substitute for medical examination, diagnosis, or treatment. I understand that massage therapy or bodywork practitioners are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and nothing said in the course of the session given should be construed as such. Because massage therapy and/or bodywork should not be performed under certain medical conditions, I affirm that I have stated all my known conditions and answered all questions honestly. I agree to keep the practitioner updated as to any changes in my medical profile and understand that there shall be no liability on the practitioner’s part should I fail to do so. I understand that any illicit or sexually suggestive remarks or advances made by me (the client) to the practitioner, or staff member, will result in immediate termination of the session and the client-practitioner relationship. I also understand I will be liable for payment of the scheduled appointment. Understanding all of this, I give my consent to receive care.
