Please answer the below question carefully! Please note PETS is a foster network of vounteers and not a 'Shelter', therefore once adopted - YOUR pet becomes a lifelong responsibility and cannot be 'handed back' or 'returned' when circumstances change. PETS will gladly assist in any way possible to re-home your pet and find a new home, but this is rarely a quick process considering the number of adult dogs already looking for homes. Please do not surrender your dog to a shelter if at all possible as this is the worst fate we can imagine for a dog used to being in a loving family. This is a lifelong commitment to an animal that places its trust in you to be a wonderful owner, so please consider how you will personally find a new home for them as one of the requirements of being a responsible pet owner. (We ae sure this will not happen, but we just need to address the possibility!)

PLEASE READ THROUGH CAREFULLY BEFORE TAKIMG YOUR COMPANION HOME - THIS IS A LEGAL DOCUMENT

It is hereby certified that in the event of this application being approved, the above mentioned applicant has adopted a companion from P.E.T.S subject to the following conditions:

1. The applicant undertakes to provide the pet with positive identification AND a contact telephone number IMMEDIATELY after adopting the new pet - such identification should at least entail a collar and a name tag suitably fitted to the pet, whilst tattoing and micro chipping would be preferable as additional methods of identification.

2. The aplicant unerstands that animals MAY NOT be chained or fstened under any circumstances and must be allowed full access to the available property/yard.

3. The applicant undertakes to fully vaccinate the adopted animal.

4. The applicant undertakes to sterilise the adopted animal at 6 months of age. Should the applicant adopt an unsterilised older animal due to unforseen circumstances, arrangements will be made to sterlise the animal as soon as possible via PETS

5. Should the applicant wish to use their own vet, proof of sterilization needs to be sent to PETS

6. The applicant agrees to random checks (using satellite or Physical ones or background checks) by PETS in oder that the sanctuary can confirm that the animals are recieveing suitable housing and care. The applicant understand that PETS has the right to remove the animals should the applicant fail to comply with rhe rules and regulations as stipulated.

7. If for any reason the applicant decide they no longer want the animal, the animal/s will not be placed in a new home without the permission/Kowof of PETS
