Without this Managent approval in writing provided back to you, no animal (even if it is not yours or your roomate's) is allowed at your premises at any time. Your lease contains the daily fee for breaking this covenant of the lease. No verbal offer, option, or perceived agreement is binding on the lease. Even if you feel you have received verbal approval of a pet, it is not valid, until you have completed this form and have received back your written approval.
Animals and/or any living thing other than humans must have written Landlord express permission to reside in or on the property at any time. Not all properties allow pets.
You may request the Landlord's permission for a pet here. Once you recieve written permission and have paid the pet deposit, pet fee, and or pet rent, the pet may occupy the premises under the rules and regulations that are in place (when you receive written permission back from the Landlord or management).
An important liability that you receive and retain along with the other obligations of brininging a pet onto the premises is, but not limited to, (1) any fleas before, during, or after introduction of your pet to the premises (there is no way to asertain where your pet picked up fleas, so your pet will always be the source of fleas, ) if you have fleas in or around your home, duplex, apartment, etc; (2) Extermination, weather invited by the tenant or the management, will always financially remain the responsibility of the tenant. (3). Animal food, toys, clothes, containers, behaviors, cleanliness, etc all contribute to fleas and bugs, to which your remain responsible.
