  • We would like on occasion to include images of your child in our informational/promotional material and venues. These may include, but are not limited to:

    • The Middletown Christian School website (www.mcssda.org)
    • Flyers, brochures, or posters advertising our school
    • Social media sites (i.e., Facebook, YouTube, Instagram)
    • Student composite pictures; headshots; photos of field trips, projects, holiday productions, and other events which may be displayed on school walls, in newsletters, newspapers, Internet postings, etc.

    No student's telephone number, email address, home address, or social media ID will be disclosed. 

  • I, (write DO) or (write DO NOT) give consent for my child(ren)'s image(s) to appear in any promotional or informational venue at Middletown Christian School.
    I further understand that any permission granted herein, or lack thereof, remains in effect until I, or another authorized guardian revoke or issue such permission via a written, signed and dated request.
