    This form is for all children registering for Faith Formation (formerly referred to as religious ed, CCD, catechsim class, etc.) for K-12th grade. If your child is in a Sacramental Prep year, or in need of RCIA Adapted for Children/Youth, that information will be made available at a later time. Children are required to be registered in Faith Formation in order to be able to prepare for Sacraments when/as they show the proper readiness for reception. If you home school for Faith Formation, please notify the Formation Office with the resources used, which must be included on the approved list for the Diocese of Fort Worth. All Sacramental Preparation must take place at the Parish!

  • Release/Indemnification Information (Form A)

    PARENT/GUARDIAN grants permission for my child/children to participate with the various programs and activities of the Diocese of Fort Worth and/or the St. Patrick Cathedral, beginning the 1st day of June, 2022 and continuing through the 31st day of May, 2023. These various programs and activities will take place under the guidance and direction of employees and/or volunteers from the St.Patrick Cathedral and/or the Diocese of Fort Worth. This permission and liability waiver will be kept on file and will accompany the child on any and all programs and activities of the Diocese of Fort Worth and/or St. Patrick Cathedral. A separate FORM B Consent to Participate and Consent to Emergency Medical Treatment must be filled out and turned in to accompany this form per each program and/or activity.I understand that as parent/guardian/conservator, I remain legally responsible for any personal actions taken by the participant named above.I agree on behalf of myself, the above named child, our/his/her heirs, successors, and assigns to hold harmless, the Diocese of Fort Worth, the Bishop and his successors, employees, agents, volunteers, St. Patrick Cathedral, its employees and volunteers from any and all claims (unless due to the negligence of the Diocese and/or Parish) for illness, injury, death and the cost of medical treatment therewith, arising from or in any way connected with my son’s/daughter/participant’s attending the various programs and activities during the dates named above.In the event any legal action is taken by either party against the other party to enforce any of the terms and conditions of this agreement, it is agreed that the unsuccessful party to such action shall pay to the prevailing party therein all reasonable court costs, reasonable attorneys’ fees and expenses incurred by the prevailing party.

    I also consent to the use of any videotapes, photographs, slides, audiotapes, or any other visual or audio reproduction (in perpetuity unless otherwise revoked by me in writing and delivered to the PARISH and by certified mail, return receipt requested, to: The Catholic Center, 800 West Loop 820 South, Fort Worth, TX 76108, ATTN: Director of Catechesis) in which my son/daughter may appear by the Diocese of Fort Worth. I understand that these materials, including websites and social media sites, are being used for promotion of the youth ministry of the Diocese of Fort Worth which may include recruitment and fundraising efforts.
  • Social Media Release

    The Diocese of Fort Worth utilizes today’s technology in a positive way to reach out to the children and youth of the Diocese, including Facebook email, and other social media. We may remove any content deemed inappropriate. All communications with any youth through social media programs by anyone representing the Diocese may be made available to any parent upon request. If you do not allow your son/daughter to text, Facebook, or use other social media, there will be no expectation that they do so in order to participate in certain ministry events. However, the Diocese cannot guarantee that photos, videos or other communications of your son/daughter from diocesan and/or parish events will not be uploaded to a social media site.

  • Diocese of Fort Worth Code of Conduct,expectations of all participants and dress code

    The purpose of this Code of Conduct is to identify personal behavior that is consistent with the purpose and objectives of the Diocese of Fort Worth. The items within the Code are based upon performance and are designed to protect the welfare of all participants.

    CODE OF CONDUCT OF ALL PARTICIPANTS                                                                              

    • Youth will respect themselves, other people, other people’s property, and church property. 
    • Youth will participate and contribute in a positive way in all activities and obey the instructions of the youth ministry, adult volunteers, and staff. 
    • During the event youth will show respect by not causing disruptions. 
    • Harassment, intimidation or bullying will not be tolerated under any circumstances. 
    • Youth will use language that reflects a Christian attitude. Youth will not use curse words or other foul language. 
    • Youth will maintain a Christian witness to the dignity of each person: no tickling, wrestling, piggyback rides or inappropriate touching (which means: do not touch anyone anywhere that a bathing suit would cover). In addition, physical displays of affection such as kissing, full body hugs, sitting on laps, and lying or sleeping next to each other are inappropriate. Being isolated or alone with another student is not allowed. 
    • Youth will wear modest clothing. No clothing that is sexually suggestive or spiritually degrading. T-shirts or other clothing with evil, vulgar, illegal, or inappropriate content will not be acceptable. In addition there should be: no midriffs, no low-cut tops, no spaghetti straps, no see through clothing, and no sagging pants to where underwear shows. Please do not wear short shorts. Shorts must follow the same guidelines as the school system. 
    • Youth will stay with the group at all times during the event and will not leave early unless approval from a parent or guardian ahead of time or in person. 
    • Youth will not possess any weapon of any kind (knife, gun, etc.) at any youth ministry activity at the church or event. 
    • Youth will not use, possess or be under the influence of alcohol, tobacco, marijuana or any other illegal drugs at any youth ministry activity. 
    • Youth will not engage in sexual intimacy or sexual harassment, and will not possess pornography at any youth ministry activity. Any illegal activity will prompt reporting to proper authorities depending upon the situation. 

    In the event the Code of Conduct is violated by a youth, proper diocesan protocols will be handled by the diocesan employee, pastor, youth minister or adult volunteer in charge. Failure to comply with the Code of Conduct will result in disciplinary action, up to and including removal from the event.

  • Faith Journey Companions

    We are always in need of help, in all areas of children and youth Faith Formation, as well as adult and RCIA. As a Catechist and/or Team Leader, you'd be part of the team of folks that will be helping to guide families on this journey. We will train and supply the necessary resources we have available. This is more than just sharing information about our great faith, it's sharing your story (your families story) and encouraging other families through these trying times in living a life for the Lord more consistently every day.
