Music lessons with Leah is a professional service that serves the purpose of educating, equipping and empowering students with skills that can enrich their life and help them experience the joy of music. I recognize that this service is treated like any other service where I am required to present payment on time. Payments are expected to be made at the beginning of the payment cycle (the 1st of the month each month or the 1st of the month for a 3 month payment period)* This field is required.

I understand that if a lesson payment is late, upcoming lessons may be put on hold until the payment is made.* This field is required.

I understand that if my child (or myself, if I am the student) is sick, I must contact Leah about rescheduling that lesson within the next 24 hours. If I do not reschedule the lesson, it will be treated as a last minute cancellation.* This field is required.

If a lesson is cancelled last minute (less than 24 hours before the scheduled lesson) and is not rescheduled, the payment for that lesson will not be refunded. The cancelled lesson must be rescheduled for a day within the next 30 days.* This field is required.
