This form indicates that on acceptance into the BCT/NABA team, the player (Parents of the player IF u18) participating authorizes BCT/NABA to act for them (the player) according to their (BCT/NABA) best judgment in any emergency requiring medical attention and the player hereby releases, exonerates, and discharges BCT/NABA and its employees, agents partners or subsidiaries from any and all action or causes or actions known or unknown resulting in any player injuries while at any of our events. Parents and players signing this form also give permission to BCT/NABA employees and agents to use the player's image and likeness via photography and video footage for the promotional purposes of both the player and the academy.

ATTENTION PARENTS- this is ELITE basketball, Your son/daughter has been invited to participate and while parents opinions which are voiced in the proper manner are valued by BCT/NABA staff and taken on board where possible, a parent of any player should never try to canvas, influence or intervene with the running of the program at any level. Continuous complaining, Chinese whispers, Gossip, canvassing of general opinion and any general behaviour that undermines the efforts of the academy staff will not be tolerated in any way. Any such breach will result in sanctions up to the dismissal of the found parents child from the program. By signing this registration form you are agreeing to the BCT/NABA parents charter and all rules and regulations contained within.
