Medical Benefits Survey

Reformed University is excited to announce that we will be adding medical benefit options for students in the very near future! Please review the two plan options below and think about what option may be best for you.

 

1. Plan 1 is priced at $300 per semester or $600 per year; this includes 1 yearly physical with labs,1 sick visit, 1 yearly dental cleaning with x-rays, and the lowest cash price guaranteed for all medical, dental and pharmacy costs.
2. Plan 2 is priced at $600 per semester or $1200 per year; this includes 1 yearly physical with labs, 2 sick visits, 2 dental cleanings every 6 months with x-rays, 1 eye exam with a free pair of glasses included, and the lowest cash price guaranteed for all medical, dental, vision, and pharmacy costs.
   
   
3. There will also be a $2500 emergency fund available for students who may receive the approval from the committee. If interested, fill out the medical benefits survey. This isn’t your regular medical benefit registration; this cash pay model has several advantages such as:

Benefit Option 2 - $600 per semester

Medical Benefit:

• (1) Yearly physical with Labs (CBC, CMP, Lipid Panels)
• (2) Sick visit

Dental Benefit:

• (2) Cleaning every 6 months - X rays included

Vision Benefit:

• (1) Eye exam - one free pair of select eyeglasses included

Pharmacy Benefit:

• Lowest cash price guarantee
