Please be as specific as possible. Designs will be finalised before the tattoo appointment, with 3 minor modifications allowed to the initial idea in this form, so do think carefully about the design before you proceed :)

50% of agreed price will have to be deposited prior to the appointment to confirm your slot. Maximum of 3 changes to appointment time allowed, thereafter deposit will be forfeited.

Allergies / Medical conditions
If you suspect that you might be allergic to tattoo inks, please let me know and schedule an appointment for a patch test. Inks to be used for your tattoo will be applied to your skin for 24 hours to test for swelling or redness~
