Baths
I understand that my pet will receive a mandatory antifungal/bacterial bath if they
have stayed for more than 5 days at a cost of $35 - $50 depending on hair length.
The baths are given the day of pick up. I acknowledge that my pet will be bathed and

I understand:

that my pet's safety is a main concern at Professional Canine and that every precaution will be taken to provide a safe and stress-free time and/or grooming experience for my pet.
that due to the intimate nature of the grooming process, or a pet's individual tolerance level for stress, strangers, or handling, some pets can become anxious or reactive during the grooming service.
that the use of common industry safety equipment such as grooming loops, muzzles, or restraints, may be used to complete the grooming service.
that if it is determined that continuing the grooming service would present a threat to my dog's emotional or physical health, and/or the safety of the employee completing the service, Professional Canine will discontinue the service.
that occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming.
that Professional Canine promotes a flea and tick free environment. If fleas and/or ticks are noted on my pet upon their arrival or while here, an appropriate preventative will be administered and I will be charged accordingly with the charges being applied to my bill.

MEDICAL:

Vaccinations
I understand that it is my reasonability to have my dog up to date on their vaccinations and Professional Canine reserves the right to turn away any boarder that is past due for vaccines.   Initials/Signature*

Kennel Cough
I understand that Professional Canine Services recommends the Bordetella vaccine prior to arriving to reduce the risk of kennel cough caused by the common bacterial agent Bronchiseptica, which can be caused by exposure to other dogs in social settings (dog parks, daycares, training classes, dog shows, kennels, etc). I agree to assume the risks and hazards that may be expected to arise from proximity and/or interaction with other animals.   Initials/Signature*

Illness/Injury
I understand that Professional Canine agrees to exercise all due and reasonable care to prevent injury or illness to my pet that could arise but not limited to upper respiratory illness due to proximity to other dogs, injury due to playing with other dogs, hotspots due to the heat and humidity, pressure point sores on the elbows and/or hocks, etc, but that these injuries and/or illness can happen. Fecal Exams: Due to the infectious nature of intestinal parasites, fecal exams may be performed on all boarders. If the exam is negative, there will be no charge to you. If your pet is found to have intestinal parasites, the appropriate medication will be instituted or dispensed to you. The charges for both the medications and fecal exam will be added to your bill. Veterinary Care
I hereby grant permission to Professional Canine Services to act in my behalf, and in my pet's best interest, by obtaining veterinary care at my expense, if deemed necessary, for illness or injury. I further agree to pay for all veterinary and other necessary services incurred by and for my pet during its stay.   Initials/Signature*

LIABILITY
The owners and employees of Professional Canine shall not be held liable for any damages to the animal arising out of or from the boarding of the animal, or that may accrue from any cause in connection with such boarding including injury during play, loss by fire, theft, running away, death, or injury during the term of this agreement, whether the animal is on the premises or not.

I agree that Professional Canine will not be held responsible for aggravation of any pre-existing medical conditions. These include, but are not limited to, heart disease, arthritis, obesity, infections and any other conditions that might be discovered.

I agree not to hold Professional canine liable for any illness or ailment that may affect my pet if my pet becomes ill during or after its visit and I agree to indemnify and hold Professional Canine harmless for any costs, damages, claims or expenses arising therefrom.

I agree to pay all costs for any property damage or personal injury caused by my pet during its stay. I agree to pay all charges on the day of pick up of my pet and I understand that my pet may not leave the premises until all charges are paid in full. I understand that any animal left for 10 days beyond the agreed date of pick up without prior notification of extension of stay, may be declared abandoned.

I agree that Professional Canine does not assume and shall not be responsible for the loss and or destruction of personal belongings left with the pet while boarding, including, but not limited to, bedding, blankets, toys, dishes, collars or leashes.

I authorize the use of photos, videos, and/or information (owner's name, general location, pet's health testing, etc) related to my pet's experience with Professional Canine . I understand my pet may be used in publications including electronic, audio visual, social media, training videos, advertising, presentations, promotional literature, stud service, litter announcements, etc. My consent is freely given as a public service without expecting consent.
