  • Thank you for choosing our clinic to board your pet. We provide quality boarding with a personal touch. Every attempt will be made to give each pet individual love and attention during their visit with us. We strive to maintain a sanitary and healthy environment for our patients.

All pets must be up to date on vaccinations (See list below). Proof of vaccinations must be on file at the time of boarding, or an exam and vaccinations will be administered upon admission at the owner’s expense.

It is our policy that all pets are clean at the time of pick up. If your pet will be with us for an extended period of time (5 days or longer) he/she will likely need a basic bath before leaving.

I understand that it is hospital policy to try and contact the owner/agent or emergency contact in the event that an emergency or illness arises. I request Foland Veterinary Services provide medical/surgical treatment deemed necessary, with fees not to exceed  $   .

  •   Boarding is charged by night. Each night that your pet(s) stays with us a fee will be charged per pet.

See the hospital and boarding policies on the back page for further details.

  •   I understand that if my pet(s) develops diarrhea due to stress from boarding or due to diet change that FVS will treat my pet(s) with Pro Pectalin a Probiotic for FOUR DAYS at no charge.
