SECTION I. HEALTH AND PRE-APPOINTMENT RESTRICTIONS

• I have read everything sent to me via email. I understand that if I didn't read the emails and all the information on the website, asking the same questions that were already sent to me or available online, it will take time away from my own appointment.
• Pre Appointment Restrictions were properly disclosed on Zhea's website.
• I have been advised and have read to follow proper pre-care instructions to avoid any discomfort or adverse side effects.
• It is my responsibility to read the pre-appointment restrictions. Should I fail to follow the pre-appointment restrictions, I understand that it could interfere with the procedure and the healed results.
• I have read everything and have a clear understanding of the service I am getting done.
• I understand that it is my responsibility to advise any concerns I have BEFORE the procedure begins.
• I am not allergic to ingredients such as glycerine, propylene, glycol, alcohol-water, antioxidant formula, lidocaine, prilocaine, benzocaine, tetracaine, or epinephrine.
• I am not and do not have the following; Prone to scarring (keloids disorder), Pregnant/Breast Feeding, Currently using Accutane, Insulin dependant diabetic, have a skin condition around the brow area (eczema, rosacea, psoriasis), have gotten a deep tan, chemical peels, botox/fillers the last 2 weeks.
• Should I be HIV positive, I will inform Zhea privately and disclose this with her in full transparency
• I understand that if I took/consumed any caffeine, alcohol, or blood-thinning drugs (painkillers) within 24 hours I may bleed more/there might be a chance that the results will fade or not appear at all.
• I understand that should I have any unknown allergic reactions it is my responsibility to consult my healthcare physician. I have also been advised and aware that it is disclosed on Zhea's website to consult my healthcare physician if needed.

SECTION II. PROCEDURE

• I was advised that there is numbing cream provided during the procedure for permanent makeup only.
• I understand that brow, lip, freckle mapping, and tattoo stencil are the part that will/might take the longest. This is also the most crucial part, where I can make suggestions/alterations before tattooing. I understand that I cannot make any changes once tattooing is done because I understand that alterations are made during the mapping/stencil process.
• I understand that there could be discomfort and pain during the procedure.
• I have told my technician any/all concerns I may have.
• Individual responses to products used for cosmetic and body tattooing may vary- it is my responsibility to seek medical attention if necessary.

 

SECTION III. AFTERCARE AND HEALING STAGES

• I am aware that the aftercare and healing stages guide is disclosed publicly on the website and it is my responsibility to follow and read it.
• I have been advised to follow proper precare and aftercare, instructions to avoid any discomfort or adverse side effects.
  
• I was advised and informed about Aftercare/Healing stages by my technician during and after the procedure. I understand that tattooing takes 7-10 (externally) and 1 month (internally) days to heal, so I might experience some discomfort as well during the healing stages.
• I understand that there might be a period where the color or pigment will look very faded but the color will resurface back for cosmetic tattooing.
• I understand that sometimes there are rare cases that the permanent makeup procedure will not stay on skin and someone might not be a good candidate for permanent makeup.
• I understand that results all vary per person, how the skin takes in the pigment, and that a touch-up might be needed. It is my responsibility to read about the aftercare and healing stages that I have access to on Zhea's website. Should I fail to follow aftercare instructions, Zhea Flores and any of her companies and the students will not be held responsible. 
• LONG TERM: I understand that this is what I wanted and asked for during this time. I understand that I may change my mind and my looks as months, years, and trends go by. I am aware and responsible for my thoughts and decisions. Should I change my mind regarding how I want my tattoos; brows, lips, fine line tattoos to look, Zhea Flores, and/or companies, and Students in training will not be held responsible for any changes I want to make due to my change of heart and thoughts on how I want it to look after getting it done. I fully understand that this is a tattoo and cannot be changed. I am aware of the sound decision I have made to permit the technicians to tattoo me and have agreed on the mapping, shape, color, stencils, and placements. etc

 

SECTION IV. PRIVACY, POLICY, AND DISCLAIMER

• I understand that the personal information I have given Zhea stays safe and will not be released anywhere.
  
• I respect Zhea's rate/pricing on initial sessions and touch-ups. I understand that all services provided by Zhea and its Respected technician/s are non-refundable and non-transferable.
• I understand that all rates are subject to change without notice and plus hst.
• I fully understand the policy page on Zhea's website and all its contents.
• I am giving Zhea permission to release before & after photos on social media and for portfolio/marketing/promotional purposes.
• I understand that Zhea is not affiliated with anyone or any company unless stated. Should I have any concerns, my first point of contact to Zhea Flores.
• I understand that breach of contract/policies, social media slandering, verbal harassment, or of any kind will not be tolerated and a penalty may be involved.(ONTARIO: Libel and Slander Act, R.S.O. 1990, c. L.12)
• I was asked a pre-screening questionnaire and I am confirming that I am not symptomatic before coming to my appointment.
• IMPORTANT: I understand that Zhea Flores implemented new protocols to protect staff, students, and clients from COVID-19. I am also aware that extra safety measures are in place before and during my appointment. Due to the unpredictable nature of the novel COVID-19, should I contract this virus, Zhea Flores and/or companies involved will not be blamed nor held responsible. Moving forward, should I contract this virus myself, I will inform Zhea Flores immediately.
• I understand that Zhea Flores has every right to politely forfeit the deposit and decline someone difficult and disrespectful for our own safety, well-being, and mental health.
• I fully understand the questions, terms, and conditions of this "Disclosure & Release Agreement" I accept to waive my rights for any claim against the technician for any reason.
  
• I release my technician/Salon/Studio (Zhea Flores/Student in training) from all liability associated with this procedure. Our company/salon/studio is not responsible for any errors.
