I also understand that the coronavirus disease (COVID-19) has been declared a worldwide pandemic by the World Health Organization. I further understand COVID-19 is extremely contagious. State and federal health agencies recommend social distancing.
I recognize that the staff at Lash & Blade LLC are closely monitoring this situation and have put in place preventative measures targeted to reduce the spread of this virus. However, given the nature of this virus, I understand that there is a risk of becoming infected with such close contact if I proceed with this service.
I understand that with receiving this service I may risk the exposure, I may or may not contract the virus or any mutation of this disease. I give my permission to the staff of Lash & Blade LLC to proceed with this service. This concept applies to sny follow up or additional services in the upcoming months.
I understand that even if I have been tested for COVID-19 and received a negative test result, the tests may not have detected the virus, or I may have contracted the coronavirus after the test. I will not hold Lash & Blade business and professionals offering the service responsible for any liability related to COVID-19 and any variation or mutation thereof.
I understand that exposure to COVID-19 before, during, or after my services may result in complications and/or delayed hearing.
If I have been tested positive for COVID-19, I understand that I have to inform Lash & Blade LLC.
I have been given the option to defer my service to a later date. However, I understand all of the risks included and would like to proceed with the service.
I am aware that I must wear a mask, bring no guests, wait in the car until your service provider texts you or calls you to let you in each time, and sanitize my hands before and after.
