I understand that my outcome will depend on the quality of my actions towards the program's instructions.I understand that it is my responsibility to mention any serious health condition. If that's the case, I ensure that I have permission from my doctor to follow any such program.

I understand that during the conduct of the suggested exercises, the reaction of my heart, lungs, and blood vessels, etc., cannot be predicted with accurac, thus risks may arise due to adverse changes that may lead to physical or/and physiological damages (i.e. heart attack, high blood pressure, or stroke.)I was given information and I understand that during any suggested training, injuries may arise such as muscle, ligaments, joints and tendons, and I have been given proper information to prevent or minimize these occurrences.I understand that such a program may appear stressful and may require taking uncomfortable action (i.e. lifting heavier at the gym).I understand that such a program may affect my mood/humour and may bring some psychological, physiological changes.

I understand that I shall solely be responsible, and the fitness trainer/group (a.k.a. Biabove Sports/F.O.W. and his trainers) shall not be held liable for any damages and injuries arising from the fitness training program.I was given information and I understand that during training, injuries may arise such as muscle, ligaments, joints and tendons; and in I have been given proper information in order to prevent or minimize these occurrences.I hereby release and fully and forever discharge the fitness trainer/group (a.k.a. Biabove Sports/F.O.W.), its assigns and agents from any claims, demands, and/or damages therein.I understand that under law 21, the instructions given are especially for a holistic wellness point of view. By default, they don't have any medical purpose, such as provided by a practitioner.

I am aware that all information obtained by the fitness group/trainer(s) from me, such as fitness records, personal information, among others, shall be treated as confidential information and that the fitness group/trainer(s) hold the information as privileged information. The information shall not be disclosed nor shared with anyone without my prior written consent.I understand that the information acquired by the fitness group/trainer may be used for statistical purposes or others so long as such information will not disclose the identity of the person.I understand that the information obtained will be used by the fitness group/trainer(s) to evaluate my progress and other internal matters.

I understand all the terms, my rights, as well as my obligations herein. I voluntarily give my consent and I declare that I was not induced or coerced into signing this form.

I am of legal age and I am legally competent to give my consent to this agreement.I am below of legal age or I am not competent to sign this agreement. My Parent/Guardian/Representative shall sign this informed consent on my behalf.
