(Please submit a separate form for each child you wish to enroll)

We at PTVM value your child's religious education and ask that you bring them to church school as often as you are able. Please let us know in the comments below if you will be able to bring your child weekly so we can plan our lessons accordingly.

We also ask any parent of our youngest littles to join their child in church school until they are able to fully participate on their own.
