Audio Recording Agreement
This form is required to be submitted each semester by any students with the accommodation of recording class, access to class notes, or any students using assistive technology that requires licenses that the Student Accessibility Services loans to students (Sonocent, Glean, etc.).

License information from Sonocent and Glean will not be sent until this form has been submitted.

Student Responsibilities

Please review and indicate that you agree to follow the statements below.
I understand that I have been approved by the Student Accessibility Services to produce audio recordings of class lectures for my study use only and no other purpose.*

I understand that I do not have permission to share my audio recordings with anyone else, unless I am a student registered with the PLUS Program, in which I can only share my audio recordings with my PLUS Learning Consultant.*

I understand that I may not copy or release any recording or transcription on class materials I have recorded, nor may I profit financially from the content I record.*

I understand that information contained in the audio-recorded lectures is protected under federal and international copyright legislation and may not be published or quoted without the lecturer’s explicit consent and without adequately identifying and crediting the lecturer.*

I understand that I am required to delete each audio recording at the conclusion of the semester.*

I understand that violating this agreement may result in the withdrawal of the authorization to record lectures as well as to a review of this accommodation or similar services in the future.*
