Arise Creations is a jewelry business that provides mentorship in leadership and professional skills for girls in 7th- 12th grade at New Life Centers in Humboldt Park. It's a great place to make friends, grow in a safe community & earn money. All participants will earn a paycheck (educational stipend) every 2 weeks for attending classes regularly. We meet 2 times per week, during class we hangout, share, learn more about our purpose and how God created us, learn professional skills and create the jewelry that we sell. You will also attend events (in person or virtual) to sell and get the word out about our jewelry. If you are interested, follow the steps below to apply. Completion of this application does not guarantee a spot, but is the first step toward joining Arise. If you are a new applicant, you will interview with us to make sure we are a great fit for each other. Finally, we will contact you to let you know if you've been accepted.

If this is your first time joining Arise, you must bring a parent to class on the first day of class. We will have orientation with parents and students during the first hour. PLEASE MAKE SURE YOUR PARENT CAN COME TO THE FIRST DAY.

All NEW applicants must interview for a spot with Arise. We want to make sure that you will be able to get the most out of the program. It is also a great chance to practice interviewing skills. PLEASE READ THE FOLLOWING, THEN SIGN TO SHOW THAT YOU UNDERSTAND AND AGREE: You understand that if you are selected for Arise, you will be expected to participate in ALL CLASSES (except 4 excused and 4 unexcused absences). You agree to communicate about absences before class. You also understand that Arise is NOT A JOB but a TRAINING PROGRAM, and that you must participate in classes with their requirements, and attend 1 selling event as part of the program. You will earn your stipend based on your attendance in class and will receive it every 2 weeks.

Both parent & student must attend to allow us to meet, build relationships and involve you as the parent in the education of your student in the Arise program. We will meet during the first class in person to go over our calendar, expectations, complete required New Life paperwork and respond to questions. (Padres/tutores y estudiantes deben asistir a la orientación con el fin de establecer una buena relación e involucrarlos en la educación de su estudiante durante el programa de Arise. Nos reuniremos en persona para repasar nuestro calendario, las expectativas, completar documentación requerida de Nueva Vida, y responder a las preguntas.)

I understand that my child will be expected to attend all Arise classes with the exception of 3 excused and 3 unexcused absences during the semester in our Stipend Program. Students will be paid for their attendance in class in the Stipend Program. I also understand that part of the Arise Creations program is mentorship which will include going on outings to continue learning and building relationships with each child. (Entiendo que se espera que mi hija asista a todas las clases de Arise con la excepción de 2 ausencias justificadas y 2 ausencias no justificadas durante el semestre en nuestro programa de cheque de pago. I understand that my child will be participating in Selling Events under the oversight of Arise staff and volunteers, and will NOT be an employee. The type of events my child will participate in may include: online virtual events, craft fairs, church events, conference booths. A permission form will be sent out prior to each "outside event" with specific details including the date, location time and selling details. (Entiendo que mi hijo participará en eventos de venta bajo la supervisión del personal y los voluntarios de Arise, y que NO será una empleada. In the event of an injury or accident, I understand that my child will NOT be covered under any kind of worker's compensation policy, as stated in the Illinois statutes relating to volunteers who are helping 501(c)3 not-for-profit corporations (like New Life Centers), and I agree not to hold Arise or New Life Centers liable for accidents or injuries that occur.(En el caso de una lesión o accidente, entiendo que mi hija NO estará cubierta por ningún tipo de póliza de compensación para trabajadores, como se establece en las leyes de Illinois relacionados con los voluntarios que ayudan a 501 (c) 3 corporaciones sin fines de lucro ( como Centros de Nueva Vida), y acepto no responsabilizar a Arise o Centros de Nueva Vida por accidentes o lesiones que ocurran.)

I give permission, in the event of an EMERGENCY where medical treatment is required, for Arise or New Life Centers staff to obtain necessary medical services of a licensed physician for my child with the understanding that they will notify me immediately.(Doy permiso, en caso de una EMERGENCIA donde se requiera tratamiento médico, para que el personal de Arise o Centros de Nueva Vida obtenga los servicios médicos necesarios de un médico con licencia para mi hija con el entendimiento de que me notificarán de inmediato.)

I understand that I am responsible to provide transportation for my child to get to and from New Life Centers for Arise. I give my permission for Arise staff to transport my child to and from events as needed. If my child is selected to participate, I give permission for any comments, photos or video taken of my child to be used appropriately for the promotion and development or Arise's programs.
