Grace Church Online Group Registration for Life Group Leaders

You will need to have the following information ready to complete this form and register your Life Group at Grace Church:
1. Your Life Group name and if the group is intended for a certain demographic. i.e. young marrieds, grandparents, young adults (21-25), etc.
2. The group leader(s) names and contact information.
3. A general description of your group's focus, including any curriculum information or other pertinent information. This is the description which will be included in the life group catalog.
4. The group meeting days and times: i.e. Once a month, 4th Sunday at 5-7p; Every Wednesday for 9 weeks at 6:30 - 8p; 2nd & 4th Sundays at 4:30-6p.
5. The group meeting location: i.e. On campus in the Total Life Center; Panera Bread Company in Shiloh.
