Settlement Deduction Authorization

Truckers Advantage Program (TAP) Service Agreement
The Owner/Lease Operator (“Driver”) signed below hereby authorizes Thomas E. Keller, Inc. (“Keller”) to deduct $17.31 per week from Driver’s settlement to pay for accounting services provided by Trucker Tax Service, Inc. (“TTS”).

The Driver and TTS acknowledge that Keller is acting solely as a third-party administrator and is in no way guaranteeing any payment or information and is only obligated to forward funds made available by the Driver to TTS under the terms of this authorization and any applicable agreement between Keller and Driver.

Such fees will be remitted directly to TTS, who will prepare/provide for the Driver, based solely upon information provided by Driver, the following services:

TAP
- Filing of individual tax return
- Live calls with tax knowledgeable consultants
- Cloud-based software application
  - Tracks revenue and expenses
  - Creates reports
The following additional services may be provided by TTS for fees not included in this agreement:
- CFO Services
- Past Tax Review
- Incorporation
- Extensions
- IRS Audit Assistance
TTS reserves the right to adjust pricing periodically. This authorization may be canceled by either party upon written notice.
