    Employee Child Care Discount Request
  • Qualified employees may be eligible for a discount on monthly tuition fee of a qualified child (“Employee Discount Program”). Please fill this form to request the discount .

    Several other conditions (including the following) are applicable for determining discount eligibility.

    By signing below, you are agreeing to the following: 

    1. This form and enrollment/re-enrollment paperwork should be filled up and provided to Kiddie Academy.
    2. Employee Discount Program is available to Full Time employees only.
    3. The discount is offered on monthly tuition fee per company policy up to a maximum benefit of $10,000 in the aggregate per family per calendar year.
    4. Employee Discount Program is available on limited seats, in select classrooms and at limited locations. The discount is provided at sole discretion of Kiddie Academy and may be modified or discontinued by Kiddie Academy at any time at its discretion.
    5. Employee Discount Program may not be combined with other offers, promotions or discounts.
    6. Kiddie Academy may choose not to award employee discount, change terms of the Discount Program or discontinue the Discount Program completely at any time at its sole and absolute discretion.
    7. You must make pickup and drop off arrangements for your child in a manner that do not interfere with your duties and work schedule.
    8. You are welcome to share your concerns or comments about the program and/or care of your child with the Academy Director. Please exercise prudence while sharing your comments with the teachers and other staff members, as in addition to a parent-caregiver relation, you have a special co-worker relationship.
    9. All policies and procedures including those listed in Parent Handbook and disenrollment notice requirements apply.
    10. Kiddie Academy reserves the right to ask you to disenroll your child from the program if there is a negative impact on your performance, your schedule or the quality of program.
    11. Child care benefits you receive under this program may be subject to taxes. You are responsible for all applicable taxes and withholdings for the benefits you receive under this program.

    If this request is approved by Director, the request will go to Area Manager and on Area Manager's approval, the request will be sent to Accounting for approval. If this request is rejected by the Director, the Employee will be notified. If request is rejected by Area Manager or Accounting, Director will be notified.

  • Notes for Directors: 

    1. Verify employee's employment tenure before approving the request  
    2. Verify that a seat is available before approving this request. Generally, One (1) seat is available for employees per 50 enrolled FTEs
    3. Employee must be wokring Full Time and in good standing to be eligible for child care discount
    4. Child's DEA must reflect the discount percentage
    5. Once the request is approved by you, the Area Manager and the Accountant, please meet with the employee and review all the terms and conditions listed at the top of this form.

    If this request is approved by Area Manager, this request will go to Accounting and on Accounting's approval the Director, Employee and Area Manager will be notified. If this request is rejected by the Area Manager, the Director will be notified.
