Please indicate if you want to pick-up medication in hospital, have your Rx processed on our on-line pharmacy, or if you wish to pick-up a written prescription to process at another pharmacy at the bottom of this form. We require 24-48 business hours (Mon - Sat) to fill prescriptions. If a prescription needs to be filled in less than 24 hours, a STAT service fee will be applied. Submitting this form does not guarantee the medication will be filled immediately.

Prescriptions filled for pick-up at Paradise Animal Hospital take 24-48 hours to process and you will be notified once it is ready.

We do not staff a dedicated pharmacy team and our priority is patient care and managing appointments. We are no longer able to send prescriptions to outside pharmacies on your behalf. If you elect to fill your prescription at an outside pharmacy, we offer the following options, please select which you prefer:
