The School Registration and Information Sheet serves the purpose of providing names of every participant (teacher coaches and students). This information is merged exactly as typed and used for planning, press releases, and certificates of participation.

Upon submission, you will see a confirmation page, and receive an email documenting the submission.
All names should be alphabetized in order by their last name. Example: Jane Doe – not ALL CAPS then John Doe.
Have students confirm the spelling of their name as they would like it to appear on certificates prior to submission.
When noting the units that apply to each student, make sure to put a comma between unit numbers.
