    I'm Alice and I'm so excited you're here! I first want to thank you for taking a few moments to complete this New Guest Color Pre-Consultation form.

    The goal with this form is to get to know you and your hair goals, which will help me determine if the service you've selected is the correct one, or if we need to make adjustments. Completing this form will also help me be able to create a plan for your first appointment and help answer some of your questions along the way.

    New Guest Color Experiences

    Below you'll find the three New Guest Color Experiences offered at Fresh Hair Studio. If you're looking for a haircut please select the Scalp Spa Haircut.

     

    Experience One:
    2.5 hours $350
    This experience is great for those who need a single process color. Below are examples of the single process colors that can be achieved in this experience:

    ​· Cover gray, blend the grow out of previous color (root touch up or color balance)

    ​· Add a small amount of dimensional color around your hairline and part line (mini foil less than 12 foils)

    ​· Add dimensional highlights and/or lowlights around your hairline up into the crown (partial foil)

    · Soften the grow out of your highlights by blurring the foil line (root smudge)

    ​· Brighten and even the tone of your highlights while adding a high shine​​​​​ with a gloss

     

    Experience Two:
    3.5 hours $490
    This experience is great for those who need more than one services done such as foiling and base color touch up or foil and gloss. (2 services are included in this experience.)

    ​· Add dimension highlights and/or lowlights all over (full foil/partial balayage touch up)

    ​· Cover gray and/or blend the grow out of previous color

    ​· Cover gray, blend the grow out of previous color and touch up your dimensional highlights

    ​· Soften the grow out of your highlights by blurring the foil line (root smudge)

    ​· Brighten and even the tone of your highlights while adding a high shine with a gloss

     

    Experience Three:

    4.5 hours $630
    Add significant change to blondes or brunettes by creating a natural looking color with an effortless grow out. Multiple services are performed to achieve your desired look.

     · Cover gray, blend the grow out of previous color and touch up your dimensional highlights

    ​· Completely change your color. This includes the combination of a full foil, hand painted color to brighten your ends, or add soft sun-kissed dimensional color

    ​· Soften the grow out of your color by blurring the lines of foil work and hand-painted work (root smudge)

    ​· A gloss to refresh shine, balance or remove unwanted tone

     · Brighten and even the tone of your highlights, or refresh shine, balance, or remove unwanted tone with a gloss

    Payment Options:

    As a client of mine you have the option to use Cash, Card, AfterPay or Cherry for your appointment(s).

    If you'd like to make installments for your services, AfterPay and Cherry are both a Buy Now Pay Later option that is available.

    •  If you've never used AfterPay or Cherry, you'll need to visit their website and sign up or apply. Both take less than 45 seconds to complete.
    • AfterPay offers 4 equal installments over 8 weeks (0% interest, $2000 max usage)
      Cherry offers 0% interest on installments of 3, 6 or 12ms depending on approval.

    Fresh Hair Studio Policies

    Cancellation Policy:
    I understand that things can come up. I value your time, and hope you value mine too. In the event you need to cancel your appointment, please be sure to notify me no later than 24 hours before your scheduled appointment. Failure to notify in time will incur a cancellation rebooking fee. Any appointment not cancelled within 24 hours of the scheduled date and time falls under my cancellation policy protocol. You will be sent a 50% rebooking fee of your scheduled services. Once the invoice has been submitted, a new appointment may be booked. If you have any questions regarding this policy please communicate them before committing to an appointment. If you no show or cancel the same day your appointment is scheduled, a 100% rebooking fee will be due before a new appointment may be scheduled. I understand life happens and in the case of an emergency or reasons out of your control arise the rebooking fee may be reduced to 50%.

    HOLIDAY CANCELATIONS:
    During the months of November & December I require 72 hours to cancel or reschedule appointments. If cancelled with less than 72 hours, a 50% rebooking fee of the services canceled will need to be paid in order to reschedule your appointment. Please know if you cancel during the months of November & December that I can't guarantee an appointment before 1/4/23.

    Saved Payment Method:
    All guests have the option to have their preferred payment method safely and securely stored on file using my POS system, Vagaro. Vagaro is a secure and PCI compliant online payment processor.

    Service Adjustment Policy:
    Your satisfaction is the highest priority. If you are not completely content, please let me know during your service or within 7 days after your service has been performed. I am committed to making any needed corrections, as long as it falls in the services discussed and agreed upon during the consultation. I will be pleased to accommodate you free of charge in most circumstances. For said complimentary service to be done, this request must be made within 7 days of your initial appointment.

    Product Return Policy:
    I do not take product returns, however if you are not in love with the products you've purchased, you may exchange them within 14 days from your purchase date. Excludes Extension Services

    Color Services:
    Please arrive with clean & dry hair or with as little product as possible in your hair for your color appointment. Depending on the amount of time booked, if you arrive with wet hair or your hair is too dirty, we may need to reschedule your appointment. If a reschedule is needed a rebooking fee of 100% will be need to be paid prior too booking.

    Extension Services:
    Please arrive with clean, dry hair or with as little product as possible in your hair for your extension appointment. Depending on the amount of time booked, if you arrive with wet hair or your hair is too dirty we may need to reschedule your appointment. If you'd prefer to have a blowout prior to your install, please let me know so I can check availability on the schedule.

    If severe matting or tangling prohibits me from beginning the service on time, a de-matting and detangling service will be charged at $140/hr.

    Extensions are a custom ordered service per guest and cannot be exchanged, returned or refunded. If you change your mind about the length or color of the original extensions after your reservation investment has been made or the service has been completed, a new reservation investment will be required. ALL Extension services are final and there are absolutely NO refunds, returns, or exchanges with extension services.

    Salon Etiquette:
    Discriminatory/Derogatory statements and actions are prohibited in the salon and will not be tolerated. I have the right to refuse service to anyone behaving improperly, intoxicated or if their state of health may influence the effects of the service.

    Confidentiality and Privacy:
    Every client has the right to privacy and confidentiality. The conversations you have with your stylist will remain confidential. Client information and email lists are confidential and will not be sold or shared with outside companies.
