First Communion & Confirmation are both Sacraments of Initiation into the life of the Church. For a child to prepare to receive one of these Sacraments, they must complete this Sacramental Preparation form, indicating they want their child to prepare for these Sacraments.
All families with children in Sacramental Preparation must be considered active parishioners through Years 1 and 2 of preparation for the child to receive the Sacraments. You can access parish status criteria at Blsac.org. The criteria is also in the bulletin.
Parishioner Status will be reviewed at this time. You will be notified by mail of your status. Letters should be received within 2 weeks after this online registration.
Active Status must be maintained throughout the school year for a child to be able to receive a Sacrament.
Sacramental Preparation is a two-year process. This process typically starts in 1st grade for First Communion and 7th grade for Confirmation.
The following students who are Blessed Sacrament parishioners should enroll:
1st & 2nd Grade (preparation for First Communion)
7th & 8th Grade (preparation for Confirmation)
Any student Baptism or 3rd graders on up who need to prepare for First Communion.
If you are a member of another Catholic parish, you will need to contact your parish about receiving Sacraments there.
For Year Two Students: (Typically 2nd/8th grade)
Families in Year 2 of formation will have retreats and meetings they will need to attend. Please review the schedules for
First Communion Year 2
Confirmation Year 2

If Year 1 of Formation was completed at another parish or catholic school, please upload a letter confirming completion of the previous year.
