iConsult Project Work Arrangements:* This field is required.I understand that iConsult Project Consultants and Project Leaders may, or may not, receive stipends to serve on a particular project team, that activities I perform for iConsult Collaborative client projects are for the purpose of gaining practical experience relevant to my program of study, that I am not an employee of the Client, and that all intellectual property I may develop in the course of my project activity remains the property of the client unless otherwise arranged. If I am asking for internship credit for the activity I do on a project, I understand that I will be responsible for the cost of any tuition fees to my college/university for any credits filed for that activity toward my degree program. I agree to not share the client's information with anyone outside of the project team. If I am an international student, I understand that the iConsult Collaborative is an experiential learning program specifically arranged with my college/university involving virtual project activities, and therefore I do not need to file CPT credits for iConsult project activities unless they are approved as a formal internship for credit toward my program of study.

Participation Commitment:* This field is required.By submitting this application to iConsult, I agree to honor my commitment to my clients, my iConsult Project Manager, and my fellow teammates; to follow iConsult procedures; and to perform the project activities that I say I will perform for the project to which I am assigned. I understand that these procedures also require me to represent my project role with the iConsult Collaborative on my resume and other professional profiles accurately, and only as the title/role has been officially assigned to me unless another description has been approved in writing by the iConsult Director. I understand that if I fail to follow iConsult procedures, or if I fail to honor my promises regarding project activities and the above commitments, I may be removed from the project team, I may not be eligible for future experiential learning activities with the iConsult Collaborative, and I may not receive official digital validation of my learning experience with iConsult that I can present as evidence of my learning to potential employers.
