We feel privileged to serve children in grades K-12th after school at Jacob's Mission Community Center. Our community center is located in East Mesa at 7830 E. University Drive, between Sossaman and the 202 Red Mountain Freeway. For the Spring 2023 Semester, we are offering twice a week after school Tutoring/Homework assistance tuesday and Wednesday. We are offering other extracurricular activity classes as well offered once a week, and we need the help of volunteers to make these programs successful. Each class is 45 minutes long.

We are looking for volunteers who can commit for one or more hours per week for the entire semester.

Note: Volunteers will be required to pass a background check, attend an orientation and trauma training prior to volunteering. We will provide this free training.

Description of Service Opportunities

We offer many different activities at the community center. Tutoring takes place Monday through Thursdays from 3-7pm and we offer extracurricular activities that occur all throughout the week, at the same times, that we need instructors and assistants for.

In return for being allowed to participate in ASA Now and Jacob's Mission Community Center's volunteer activities and all related activities, including any activities incidental to such participation (“Volunteer Activities”), the undersigned Volunteer or Parent/Legal Guardian of Volunteer if Volunteer is under age 18 (hereafter referred to using “I”, “me”, or “my”) legally agrees to the following terms and conditions related to the request to attend or allow Minor(s) named above (“Minor”) to attend ASA Now and Jacob's Mission Community Center programs, events, and related activities. If Volunteer is under age of 18 (hereafter referred to using "Minor", I give permission for "Minor" to attend the Program or Event with or without me in attendance, without restriction or limitation. 

I accept full responsibility and assume the risk and liability for any and all damages and injuries that I might suffer or sustain as a result of my attendance at the Program or Event or any areas surrounding the Event. I hereby release ASA Now, Jacob's Mission Community Center, affiliates, subsidiaries, suppliers, related companies, advertising and promotion agencies or partners, sponsors, owners or lessors of the Event site, and any of their respective officers, directors, employees, representatives, agents, volunteers and contractors (collectively, the “ASA Now”) from: (a) any and all liability relating to, or that may arise from, my attendance at the Program or Event and/or my decision to allow Minor to attend the Event; and (b) from all liability and obligation to me and/or Minor arising out of or in connection with the exercise of the rights granted herein including, without limitation, from any liability for violation of rights of privacy, publicity, defamation, emotional distress or any similar right.

I hereby understands and if applicable agree on behalf of Minor that any legal terms and conditions, limitations, waivers, releases, or similar Event legal requirements (whether posted at the Event, printed on tickets, presented during the ticket purchase process, program or event sign-up or otherwise legally binding on Program or Event attendees) shall be applicable to me and I shall be legally responsible for any violation of any such terms and conditions.

By attending this program or event, I consents to interview(s), photography, audio recording, video recording and its/their release, publication, exhibition, or reproduction to be used for news, web casts, promotional purposes, telecasts, advertising, inclusion on websites, social media, or any other purpose by ASA Now or Jacob's Mission Community Center. Images, photos, and/or videos may be used to promote similar ASA Now and Jacob's Mission Community Center events in the future, highlight the event and exhibit the capabilities of ASA Now. I release ASA Now, Jacob's Mission Community Center, its officers and employees , and all persons involved from any liability connected with the taking, recording, digitalizing, or publication and use of interviews, photographs, computer images, video and/or sound recordings. By participating in the event or entering the event premises, I waive all rights I may have to any claims for payment or royalties in connection with any use, exhibition, streaming, web casting, televising, or other publication of these materials, regardless of the purpose or sponsoring of such use, exhibiting, broadcasting, web casting, or other publication irrespective of whether a fee for admission or sponsorship is charged. I also waive any right to inspect or approve any photo, video, or audio recording taken by ASA Now, Jacob's Mission Community Center or the person or entity designated to do so by ASA Now or Jacob's Mission Community Center. I have been fully informed of my consent, waiver of liability, and release before entering the event.

Liability Release - I hereby release ASA Now, Jacob's Mission Community Center, their agents, employees, personal representatives, volunteers, affiliates, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, arising out of or in any way relating to injuries and damages, as a result of being present on ASA Now and Jacob's Mission Community Center property. I recognize that there may be inherent risks associated with participating in and/or attending this program or event. I acknowledges and fully understands that I will be participating in activities that may or may not involve risk of serious injury, permanent disability, property damage and/or death. These risks may result not only from my own actions, inactions, or negligence, but also from the action, inactions, or negligence of others. Further, there may be other risks not known to me, or not reasonably foreseeable, such as disability or death. By participating or attending this program or event,I assume all the foregoing risks and accept personal responsibility for any damages following any such injury, permanent disability, property damage, or death. I understand that ASA Now and Jacob's Mission Community Center does not provide medical coverage to a participant, attendee or volunteer if injured while participating in the event or activity. Any medical costs incurred as a result of this program or activity will be my financial responsibility.

I release, waive, discharge, and covenant not to sue ASA Now, Jacob's Mission Community Center, their board members, officers, agents, emplyees, personal representatives, affiliates or volunteers from demands, losses, or damages on account of injury, including death, property loss or damage to property, caused or alleged to be caused in whole or in part by the negligence of any person or otherwise, for myself, if any, and our heirs, successors, and assigns.

In consideration of the opportunity afforded, with full knowledge and acceptance of the risks associated with this event; and with full understanding of the above issues/conditions and risks, I hereby release, indemnify and holds harmless ASA Now and Jacob's Mission Community Center from all form and manner of risks inherent in, and from all claims, suits and demands of any nature arising from participation in said event, or activities.

You have been fully informed of your consent, waiver of liability, and release before entering, attending, participating in, or volunteering for this event. 

I understand that this document is intended to be as broad and inclusive as permitted by the laws of the
state in which the Volunteer Activities take place and agree that if any portion of this Agreement is invalid, the remainder will continue in full legal force and effect.
