Application for Multicultural Center Access Privileges

I am applying for access privileges to the Multicultural Center. I understand that:

• Food inside the Center is available for student consumption
• Students will thoroughly clean up after themselves.
• Food marked off limits is not available and will not be touched.
• The offices are off limits without prior approval.
• Business machines are not available for student use.
• Sexual activity, public drunkenness and offensive language are not permitted.
• Guests are welcome in the day and for public events. Guests may be brought into the Center when it is closed by a host with key card access. Both parties must sign in and out in the log book.
• Everyone accessing via their electronic key card must sign the register when both entering and exiting. Anyone not signing the register will be asked to leave.
• Violations of the above may result in suspension or permanent revocation of access privileges.
• Under no circumstances are the following permitted:

✓ Carrying or consuming alcohol
✓ Possession, use or sale of illegal drugs
✓ Smoking
✓ Stealing or unauthorized use of supplies
✓ Vandalism or intentional damage
✓ Physical, emotional or spiritual violence carried out on any person
✓ Possession, use or sale of firearms or weapons

I have read the Multicultural Center Access Policy and agree to abide by its provisions. I understand that violations of the rules of the Center will result in loss of access to the Center. Violations of Center rules may also result in referral to Campus Safety and possible judicial charges.
