There is a $15 audition fee required of each dancer who is auditioning for CCB productions.
CCB Students: A performance fee of $45 will be required of dancers who are invited to join the cast and are enrolled in a level appropriate ballet class at CCB for the Fall 2021 session.
Outside Dancers: A performance fee of $195 will be required of dancers who are invited to join the cast but who are not enrolled in a level appropriate ballet class at CCB for the Fall 2021 session. Payment installment plans for the performance fee will be available.
Full-time college students: A performance fee of $25 will be required of dancers who are invited to join the cast and are full-time college students.
**Dancers cast in certain adult lead roles will be exempt from a performance fee.**
Attendance at all rehearsals and performances is mandatory unless pre-approved. This includes Sunday rehearsals and all tech week rehearsals. A Christmas Carol will be performed at the Gordon Center for Performing Arts twice on December 18th. Mandatory tech/dress rehearsals will take place in the studio and at the theater on December 13th-17th, and other routine rehearsals will be held on Sundays after 1:00pm. Extra rehearsals on Saturdays may be added depending on dancer availability.
All anticipated absences MUST be noted earlier on this form. If unforeseen circumstances arise, and you must miss a rehearsal that has not yet been noted, please give us at least two weeks notice. Email firstname.lastname@example.org to let us know. Last minute absences, unrelated to COVID-19, will not be tolerated and will result in expulsion from the production.
In case of major illness or an emergency, dancers will be excused from rehearsals but are expected to notify us beforehand. In case of minor injury, dancers will be expected to attend rehearsals to observe.
Dancers must stay home if they are not feeling well or anyone in the household has any COVID-19 symptoms. Dancers who test positive or are knowingly exposed to COVID-19 must perform a 2-week self-quarantine (or produce a negative PCR COVID-19 test at least 5 days after exposure) before returning to the studio. Any dancers with minor symptoms and/or who are quarantining will be expected to attend rehearsal virtually via Zoom.
When a rehearsal is missed, dancers are expected to be caught up on choreography before the following rehearsal.
All people, regardless of age or vaccination status, must wear a mask at all times at CCB.
Dancers must be ready to dance at their rehearsal time. Please do not be late.

Appropriate ballet attire should be worn for all rehearsals. If you have any questions about what this means, please contact us.
The rehearsal schedule will be posted weekly on the CCB website (www.charmcityballet.com) under the “Performance” tab. Rehearsal schedules will not be emailed out, so it is your responsibility to check the website and find out when you are needed. The rehearsal schedule will change weekly.
Though not mandatory, dancers (who are 14 and older) are encouraged to attend our drop-in class to be used as a warm-up before rehearsals. CCB cast members are given a discounted rate, and class goes from 11:30am - 1:00pm each Sunday.
All costumes are the property of CCB and must be returned. Performers will be held financially responsible for any damage resulting from improper care.
While costumes will be distributed by CCB to performers, cast members will be responsible for providing their own tights, shoes, undergarments, and stage makeup. More information regarding costuming will be provided closer to the performance dates.
Performers will be expected to maintain a professional attitude throughout the entire rehearsal and performance process. Disrespect toward any CCB staff or cast member, or the theater and it’s staff, will not be tolerated.
I certify that I am over the age of eighteen years, or am a parent/legal guardian, and authorized to sign this release, and that I have read the CCB Production Policies & Info and fully understand the contents, consequences, and implications of signing this document.
General Waiver & Release of Liability
I certify that I am/my child is capable of physical exercise, and I have completely documented any health or other concerns above. I agree that my child or I will participate in dance related activities given by Charm City Ballet upon the following conditions:
I understand that dance is a physical activity and that there is a possibility of injury in participating in dance related activities. Upon signing this form, I agree not to hold Charm City Ballet or any faculty member, employee, or guest teacher liable for any injury sustained or illness contracted while I/my child is a student. I release Charm City Ballet and all of its officers, directors, and employees from any and all liabilities for any injury or illness arising out of participation in the dance related activities offered by Charm City Ballet, including but not limited to personal injuries, including death, and loss of or damage to property occurring on or off the School’s premises. I assume all risks associated with participation.
I agree to participate in any and all activities including but not limited to rehearsals, auditions, performances, publicity activities, photographs, and videotaping in connection with Charm City Ballet’s auditions, rehearsals, and performances. Charm City Ballet shall have the right to use any such recordings and photographs as listed above for its own promotional and marketing purposes, including online promotion on the Charm City Ballet website and through social networking websites.

Waiver of Liability Relating to COVID-19
I hereby confirm that the Participant will never enter Charm City Ballet with an elevated temperature or while exhibiting any symptoms of COVID-19, and the Participant is in good health and able to participate in classes, events, or other activities at Charm City Ballet.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and knowingly and voluntarily assume the risks that I, or my child(ren), may be exposed to, or infected by COVID-19 by attending classes/events at Charm City Ballet, and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death. I understand that the risks of becoming exposed to or infected by COVID-19 at Charm City Ballet may result from the actions, omissions, or negligence of myself and others, including, but not limited to Charm City Ballet and their directors, teachers, employees, and volunteers, fellow participants and their families, and all employees/staff of Cushman & Wakefield properties. In addition, Charm City Ballet maintains the right to, at any point, by state mandate or otherwise, cancel or postpone any and all show-related activities, including rehearsals and performances, if it is determined that doing so would maintain the health and safety of the dancers, their families, and our staff. In this situation, production fees may not be fully refunded.
I knowingly and voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself or my child(ren) including, but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)s attendance at Charm City Ballet. On my behalf, and on behalf of my child(ren)s, I hereby release, covenant not to sue, discharge, and hold harmless Charm City Ballet and their respective directors, teachers, employees, volunteers, agents, representatives, successors and assigns of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Charm City Ballet and their respective directors, teachers, employees, volunteers, agents, representatives, successors and assigns after participation in any classes, events, or other activities at Charm City Ballet.
COVID-19 CCB Agreement
I want to do my part to help CCB keep myself, my child(ren), other dancers, the instructors, other families, and everyone else at the studios as safe as possible during the COVID-19 pandemic and beyond it. I have read, understood, and agree to follow the following policies and procedures.
I understand and agree that: - I am aware that all people, regardless of age or vaccination status, must wear a mask at all times at CCB. I also understand that outdoor breaks (without masks required) will be given when necessary.- I will drop my dancer off and pick them up at the designated entrance to the studio. (Detailed info will be emailed and can also be found here.) If I drive myself to the studio, I will follow all entry/exit procedures.- The CCB lobby will remain closed. (For younger dancers, please let us know ahead of time if you must accompany your child into the building. Only one parent or nonparticipating individual will be allowed to enter the building at the main entrance for drop off of younger children.)- I will arrive, or I will have my dancer arrive, dressed and ready for class as the dressing rooms will be closed. (Bathrooms are available.)- I will support the social distancing standard of at least 6' outside and inside the facility (and educate my child on this protocol).- I/my dancer will have regular opportunities to use their hand sanitizer, starting with prior to entering the building. - I/my dancer will use the restroom and wash their hands thoroughly before leaving home and while at the studio as needed.- I agree to stay home, or keep my dancer home, if they are not feeling well or anyone in the household has any Covid-19 symptoms such as coughing, has a temperature over 100.4 degrees, fever, chills, body aches, or sore throat. - I understand and agree that if I, or my child, tests positive or is knowingly exposed to Covid-19, I/my child must perform a 2-week self-quarantine (or produce a negative PCR Covid-19 test at least 5 days or more after exposure) and I/my child will not be allowed to enter the studio during this time. Students may participate in rehearsal virtually during this time period. (CCB COVID-19 Protocols can be found here.)- I understand that at any point, due to state mandate or otherwise, CCB has the right to cancel or postpone any and all show-related activities, including rehearsals and performances. In this situation, production fees may not be fully refunded.- I understand and agree that these procedures may change and evolve over time and that I will follow any new standards required by the State of Maryland and/or CCB.- I understand that the instructors and everyone at the studio will make a strong effort to maintain social distancing but that there will be times when incidental contact and less than prescribed physical distancing will occur. - I further understand that I am voluntarily choosing, or allowing my child, to participate in programs and activities offered by CCB knowing that even with increased sanitation and enhanced hygiene protocol, it is impossible to provide a completely sterile environment so the risk of Covid-19 exposure and other viruses may exist. I accept that risk.
I HAVE READ THE ABOVE AGREEMENTS, FULLY UNDERSTAND THEIR TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING HEREIN AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE AND WAIVER OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
