This application form is to be used for all submissions of grant/loan requests to Passion 4 Youth, Inc (P4Y).
There are 2 type of grants offered from Passion 4 Youth(P4Y): 1) PG - Player Grant: Athletes with financial constraints whom are pursuing an opprotunity to enhance their future in sports. 2) GG - Group Grant: Teams or organizations whom need financial assistance to provide opprotunities to athletes, youth in need and/or to provide tools/resources.
