Patient Acknowledgement: COVID-19 Pandemic Dental Risk
新冠肺炎疫情期間牙科診療風險須知
1. I understand the novel coronavirus causes the disease known as COVID-19 and that it is currently a pandemic. I understand the novel coronavirus virus has a long incubation period during which carriers of the virus may not show symptoms and still be contagious.
本人理解目前正處於新冠肺炎（又名COVID-19）疫情期間，並理解新冠肺炎病毒潛伏期間，感染者可能產生無症狀傳播。
2. I understand the federal and provincial governments have asked individuals to maintain social distancing of at least 2 meters (6 feet) and I recognize it is not possible to maintain this distance while receiving dental treatment.
本人知悉聯邦及省政府對於個人保持兩米社交距離之要求，並理解於牙科診療過程難以維持此社交距離規範。
3. I understand that due to the visits of other patients, the characteristics of the novel coronavirus, and the characteristics of dental procedures, that I have an elevated risk of contracting AND SPREADING the novel coronavirus simply by being in the dental office.
本人理解牙科診所可能因其他看診患者、新冠肺炎傳播特性及牙科診療性質，而提高暴露於或散播新冠肺炎病毒環境之風險。
