Volapalooza 2019 - Volunteer Honoree Nomination Form

Thanks for honoring local volunteers at our
2021 Virtual Volapalooza!

It's easy to nominate a volunteer for recognition and here's what you need to know for this year's event.

Organizations from all sectors - nonprofit, company, community or faith-based group, municipalities - can nominate volunteers serving DuPage County.
Complete this Volunteer Honoree Form as soon as possible for every honoree you nominate
Double-check the spelling of all volunteer names. We use this information you provide in a multimedia presentation
Attach a photo of each volunteer and rename the file to include the volunteer's name before uploading the photo, such as JoeSmith-OrgName.png or JackieHall-OrgName.jpg - it helps us to match the volunteer name to photo.
