If you are wanting a hair color service, are you open to other color options, if the color you desire is not going to be realistic or healthy for your hair?

When was the last time YOU colored your hair at home using NON PROFESSIONAL HAIR COLOR (box hair color)?

Have you had any allergic reactions in the past after coloring your hair or do you need to use medicated hair products? (Our color & hair products contain nuts.)

How often have you been getting a salon service done? Example: every month, every 5-6 weeks, two times a year etc.

What brand of hair products are you currently using?

Stylist Wanted (Some stylists aren't able to take new guests at this time and some stylists have no Saturday hair color appointments available. We can make recommendations for the best stylist for your hair goals once we see this form.)

What is the budget for your service? A new guest partial custom lightening only service with one of our stylists ranges between $450-$525 for the first visit, depending on your current hair color, the color you are wanting and the amount of hair you have. Your maintenance appointment will be less. A root touch up appointment with a gloss starts at $135. A haircut with blow dry is $110 or blow dry only is $65 and is an additional fee. Extension pricing varies.

I understand that all new guests are required to schedule a consultation only appointment prior to a service appointment. A service appointment can only be booked during your consultation and we CANNOT combine a consultation & service appointment together. They need to be two separate appointments. You may be asked to participate in a strand test ($25) to see how we can proceed with future color services. If you do not wish to proceed with a stand test, we cannot provide any service.

What is your availability for a service and how soon do you want an appointment? We are accepting new guests for Tuesday-Friday 10am-4pm. Immediate, evening or weekend appointments are not available at this time.

Due to liability issues, bringing guests, including children in our salon, without appointments poses unique and previously unforeseen challenges. Our salon can be a very dangerous place. For children, we cannot assure their safety in the salon. We have hot tools, chemical products, sharp objects and no facility to care for your children while you are in our hands. We kindly ask that you do not bring any guest including adults or children to accompany you during your appointment.
