I, THE PARENT/GUARDIAN OF PARTICIPANT, hereby authorizes Inspiring Young Minds (“IYM”) to use my child’s likeness in any photograph/video in any and all of its publications, including but not limited to, all IYM printed and digital publications. I understand and agree that any photograph/video using my likeness will become the property of IYM and will not be returned.

I acknowledge that since my participation with IYM is voluntary, I will receive no financial compensation.

I hereby irrevocably authorize IYM to edit, alter, copy, exhibit, publish or distribute this photograph/video for purposes of publicizing IYM’s programs or for any other lawful purpose. I waive the right to inspect or approve the final product, including a written or electronic copy, wherein my likeness appears.

I hereby hold harmless and release and forever discharge IYM from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this release. Waiver of Rights
This Agreement contains a waiver of rights by the people signing it. PLEASE READ IT CAREFULLY. If you have any reservations or questions about signing this document, please do not sign it.

This Agreement is entered into by and between Inspiring Young Minds (“IYM”) and THE PARTICIPANT and/or his/her parents or legal guardian, if the Participant is under 18 years of age. In consideration of the Participant being able to participate in activities promoted by IYM, the Participant, or the parents or legal guardian of the Participant agree: 1) to release, discharge and hold harmless IYM and its members, officers, directors, employees and agents individually and collectively from any and all liability, demand, claim or action in law or equity arising out of or in consequence of participating in any activity promoted by IYM, including specifically but without limitation any injury, disabling event and/or death; 2) to assume all potential risks associated with participating in an IYM activity; and 3) to authorize IYM to obtain emergency medical treatment in the event of injury and that IYM shall not be required to maintain medical or hospitalization coverage to pay for such treatment.
