Students, who meet the following criteria, may request permission for a leave of absence:
- good academic standing;
- completed at least one full term of enrollment at Reformed University;
- has an involuntary need to be absent from the university for a period of time; and
- fully expects to return to complete requirements for the degree.
This Leave of Absence Request Form should be filed by the last working day before classes start in the semester/session the leave will begin in addition to a letter of explanation and the official letter confirming status.
