After you submit the form you will receive a copy of your request via email. You will also receive another email with Delivery Agreement and ID request instructions; if you did not receive the second email or you want to speed up the process you can download the agreement document from the same web page where you found this form. You need to fill out, sign, and email the completed form along with a copy of proof of military service: VA ID, DD-214, retired military ID. etc (DON'T SEND PHOTO OF DRIVER"S LICENSE and DON'T SEND PHOTO WITH YOUR SOCIAL SECURITY NUMBER SHOWN) to the email listed on the agreement form.
Once you submit both forms you need to be accessible by email and phone so we can schedule your delivery. We try to complete and deliver your request quickly and you need to respond within two (2) weeks. Failure to respond to our calls and emails will result in you going to the back of the line and delaying your delivery; we may even cancel your request if we can't reach you.
