    (Form rev November 1, 2025) Please read before you fill out this form. All information is required to be filled out. If you need to make additions, corrections or changes YOU MUST FILL OUT A COMPLETELY NEW FORM AND WE WILL DELETE YOUR FIRST REQUEST. Our furniture request email address is veteran_request@sofasforservice.org. We serve veterans-in-need in Wisconsin and eastern Minnesota within about a 2-hour drive from our warehouse in Altoona, WI. We generally don't provide services outside of this area. We will try to fill your request as soon as possible but it is possible that your request may take up to 2 weeks or longer to process. We are a volunteer run organization and have a large number of veterans-in-need requesting furniture and household goods. You can receive your items sooner if you are able to transport your items yourself by “shopping” at our Altoona, Wisconsin warehouse in person. The warehouse is staffed on Wednesday and Saturday mornings from 9:30 AM to 11:30 AM. If you want to shop at the warehouse you will need to set up an appointment.
  • Winter 2024-25 operation policies

    There are a few things you should be aware of when we are in the winter season - which means snow and ice.

    A. Weather and road conditions may force us to delay or even cancel service if we feel it is unsafe for our drivers.

    B. Your building or house, when we deliver, must be free of snow and ice and give us safe passage to the front door - this includes driveways, sidewalks, steps and if we have to park on the street it must be plowed. We reserve the right to not deliver if the above condition is not met.

    C. If you are unsure that the area is clear enough for delivery please email a photo(s) of the area(s) where we will deliver to veteran_request@sofasforservice.org

  • It might take 2 weeks or longer to fill your request, given the number of requests we receive and that we are a volunteer organization without office staff.

    What is the ADDRESS the Veteran is MOVING TO? Be accurate, this is where we will deliver your household items to. (Please include "Street", "Boulevard", "Court", "Road", etc.) 

  • Service Confirmation
    We need proof of military service from the Veteran before we start on this request.

  • You will need at least two people (include yourself if able) to help unload items once we arrive at your delivery address - our drivers' tasks is to deliver and not load/unload. We will call you on your delivery day and verify you have at least 2 people (include yourself if able) to help. If you don't have help we won't deliver. If we get to your place and you don't have help we will not deliver your items. Your help canceling on delivery day is not a valid excuse.  If you have trouble finding someone to assist you then we suggest you contact the local County Veteran's Service office, local VFW and American Legion Posts or a similar organiztion.  It is your responsibility to ensure help is there when we arrive.

  • We can usually provide most items a veteran in need and his/her immediate family find necessary to set up a household. While we try to keep this listing up-to-date our inventory is dependent on donations and fluctuates weekly - if your requested item(s) is not available we will do our best to get you a close substitute from our inventory.

    PLEASE DO NOT SELECT MORE THAN ONE TV.  OUR POLICY IS ONE PER VETERAN HOUSEHOLD. 

    Important Note: We DO NOT provide a traditional style mattress and box spring; instead you will receive a Mattress-In-A-Box and 14" high steel slat bed frame, both of which require you to set up and/or assemble. Once you unbox and roll out the mattress you must wait at least 24 to 48 hours (longer is better) before sleeping on it to allow full inflation. Sleeping on the mattress before it reaches full inflation may cause a permanent dent/depression in it.

  • After you submit the form you will receive a copy of your request via email.  You will also receive another email with Delivery Agreement and ID request instructions; if you did not receive the second email or you want to speed up the process you can download the agreement document from the same web page where you found this form. You need to fill out, sign, and email the completed form along with a copy of proof of military service: VA ID, DD-214, retired military ID. etc (DON'T SEND PHOTO OF DRIVER"S LICENSE and DON'T SEND PHOTO WITH YOUR SOCIAL SECURITY NUMBER SHOWN) to the email listed on the agreement form.

    Once you submit both forms you need to be accessible by email and phone so we can schedule your delivery. We try to complete and deliver your request quickly and you need to respond within two (2) weeks. Failure to respond to our calls and emails will result in you going to the back of the line and delaying your delivery; we may even cancel your request if we can't reach you.
