Informed Consent and Acknowledgement
I hereby give my approval for my child’s participation in any and all activities prepared by Broken Leg Production during the selected program. In exchange for the acceptance of said child’s candidacy by Broken Leg Productions., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Broken Leg Productions. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.
In case of injury to said child, I hereby waive all claims against Broken Leg Productions. including all staff and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
CAST CONTRACT
Welcome to Broken Leg Productions and our SPRING 2021 programming! We are so excited about these shows and the opportunity to bring quality theater to our students! As a member of the cast of these productions, BLP requires cast members to commit to the following policies. Checking off on the registration will serve as an electronic receipt of signature for these policies. Any questions or issues with these policies should be addressed prior to auditions. Thank you for your time and we look forward to creating theatrical excellence with you.
All cast members must be age of eligible age as of September 2020
All cast members must attend all rehearsals they are available. Each student will be allowed two conflicts over the course of rehearsals. These conflicts cannot be during tech week, unless directly approved by BLP before the rehearsal process starts. Obviously, emergent situations and illness will be an exception which must be communicated with BLP. All performances must be attended by all performers. Call times for rehearsals and performances must be adhered to; repetitive tardiness or leaving early will not be tolerated.
All cast members agree to work together. BLP has a strict no bullying policy in place which will be reviewed at the start of rehearsals.
Each cast member will be provided with a libretto/vocal book. This can only be written in with pencil and must be returned in good condition at the end of the session. If lost or damaged the cast member is responsible for a $25 replacement fee. Until paid they will not be able to participate in any further BLP productions.
All cast members will bear ultimate responsibility for their costumes. BLP in return commits to providing access to our costume resources and staff for alterations, manufacture (when needed), maintenance, and coordination. All cast members must supply personal eye makeup. Cast members further agree to consult with the director or production manager prior to any drastic changes in appearance (haircut, hair dye, etc.).
All cast members will be respectful of all props, sets, technical equipment, etc that they may encounter. If a cast member breaks any equipment, they may be charged for replacement depending on the circumstance.
All cast members commit to participate in any publicity and fundraising efforts. All cast members grant BLP permission to use cast members’ images in any official publicity pieces. Publicity pieces include, but are not limited to, news releases, publications, videos, website use, Facebook use, and photo sale at performances of the production.
All cast members commit to follow the rules and regulations of locations used for rehearsals, performances, or other production related activities. Cast members also agree to treat said spaces with respect and care, acknowledging that we are guests in these spaces and it is a privilege to use them.
Students agree to be respectful of the rehearsal process. We have limited time to get a lot done. Students must come prepared with any materials needed and remain focused. Technological devices should be silenced and not used when working at rehearsal. Students agree to adhere to off-book dates and other deadlines as set forth by the production team
If a student withdraws from the production prior to auditions they will receive a full refund less a $50 withdrawal fee. If a student withdraws from a production after auditions they will not be refunded.
Recommendations
Bring script, pencil, and proper footwear/attire to rehearsal.
Be sure to rehearse and review lines/staging/music/choreography outside of rehearsal.
Feel free to bring a snack to rehearsals, but be sure to clean up after yourself. We recommend bringing water to rehearsal.
We recommend all students exhibit proper hygiene due to the closeness of students. There is a lot of active movement at rehearsal; wearing deodorant is encouraged.
If a student contracts a contagious illness, please communicate with BLP A.S.A.P.
