HALL OF FAME – SUPERIOR PERFORMANCE APPLICATION

California USBC Local Associations may submit nominations for Superior Performance applicants. All nominations are considered confidential. If an applicant is not selected, but deemed qualified, their application shall be retained for consideration up to 3 years. Updates are encouraged by the nominator. NOMINATION DEADLINE: JANUARY 15. Qualifications for Superior Performance shall include:
1. A minimum of 15 years as a California USBC* member (unless career has been interrupted or shortened due to illness or disability)
2. At least 45 years of age.
3. A minimum of 12 years bowled in the California USBC* State Championships (unless career was interrupted or shortened by illness or disability)
4. A minimum of 1 first place finish in the California USBC* State Championships, California State Masters or the California State Queens.
5. Please provide a recent photograph
*California USBC includes CWBA and CSBA
