HALL OF FAME – MERITORIOUS SERVICE APPLICATION
California USBC Local Associations may submit nominations for Meritorious Service applicants. All nominations are considered confidential. If an applicant is not selected, but deemed qualified, their application shall be retained for consideration up to 3 years. Updates are encouraged by the nominator. NOMINATION DEADLINE: MARCH 15. Qualifications for Meritorious Service shall include:
A minimum of 20 years of certified bowling in California
A minimum of 15 years of bowling service and promotion in California
At least 50 years of age

Number of years as a member of California USBC (includes CWBA, CSBA and CSYABA). Please list years.
Summary of the outstanding service in the promotion of bowling – local, state or national
List any offices held in League, Local Associations, State Association or National USBC.
List activities that promote the growth of the sport.
Has the nominee ever been suspended or re-rated by USBC in the past? If yes, you must submit a paragraph explaining the circumstances behind such action.
