I understand that the bath/groom will be attempted without sedation, but on the chance that sedation is needed, I authorize Dr. Brad Nunley and staff to perform sedation on my pet. I understand the risks associated with sedation(vomiting, lethargy, death)If my pet cannot be bathed/groomed with being sedated, I DO NOT want my pet sedated and I will receive a call from Animal's Choice Vet Clinic to pick my pet up without being bathed/groomed.
