GENERAL POLICIES (READ TO CLIENT BEFORE LEAVING)
CAMC makes every effort to keep your pet safe while in our care, but some things are out of our control. We need to make you aware of these policies:
· Our boarding rates are based on a 24 hour period. This means if your pet(s) check into boarding at 10 am, your checkout time is 10 am on the day of checkout. If you pick up after that checkout time, you will be charged an additional day of boarding.
· We are not staffed 24 hour a day.
· We take all dogs outside at least 3 times each day. We are not responsible for any bee stings, bug bites or injuries that occur.
· We use blankets and/or towels in the boarding suites and condos unless you ask us to remove them. We are not liable for any injury or illness if your pet chews or eats them.
· We allow clients to bring toys and bedding from home. We are not liable for any injury or illness if your pet chews or eats them, and we are not responsible for replacement of any damaged items.
· We require certain vaccinations to be current prior to checking your pets into our boarding facility. For your pet(s)'s and our team members' safety, your pet will not be accepted for boarding without record of these vaccinations being current (not expired): Dogs: Rabies, Distemper, Bordetella, Canine Inluenza N2/N8; Cats: Rabies, FVRCP.
· If your pet has a medical emergency while here, we will try to contact you. If we cannot reach you immediately, we will try to stabilize your pet. There may be additional charges incurred and by leaving your pet here, you agree to pay for these charges.
The CAMC receptionist, has explained the boarding requirements, and I understand and agree to follow the hospital's policies. I agree to pay all boarding charges incurred, and I authorize and agree to pay for all veterinary services requested and/or required on the attached Boarding Preventive Care Report(s).
