As part of Enrolment at our Service, we require acceptance of the following items in order to receive Government Funding on your behalf. Acceptance of these items will be via a Complying Written Agreement (CWA) for Child Care Subsidy (CCS) purposes. The CWA can be viewed in your child's profile in Xplor.

I understand that Sherwood State School OSHC will notify me of the days my child/ren have been allocated bookings. I understand if they cannot be confirmed they will be placed on a Waitlist and informed should a spot become available. I understand I must contact Sherwood State School OSHC if I wish to change/cancel bookings and give the required notice to avoid charges (refer to Cancellation Policy in the Family Handbook).

I confirm:

• My details and my children's details are correct.
• I have agreed to days of care with the service and understand the start and end times of these sessions of care.
• That care may be provided on a casual basis where available upon my request.
• I understand I am liable to pay fees for the care of my child and agree to the Cancellation/Policies as detailed in the Bookings and Fees policies.
• I have reviewed my child/ren's Xplor profile and all contact details, including Emergency Contacts and medical information, are up to date.
